This is a graphic organizer I used on a regular basis with my 4th grade special education class. It was extremely helpful in getting them into the routine of thinking about these four questions as they read because they knew they would be asked later.

Directions: Re-read and use the pictures to help you remember the 4Ws of the book.

Questions:

Who are the main characters? Describe them.
Where does the story take place? Describe the setting.
When does the story take place? Describe the time period.
What is this story about? Describe the main idea.

It was given to them occasionally for in-class independent reading, one of their reading homework assignments and placed in holiday packets and the "If You Finish Early" basket.
